Diploma Replacement

To order a replacement diploma, please do one of the following:

• WEB: Download the Application for Diploma Replacement from http://www.registrar.arizona.edu/graduation/diploma.htm

• PHONE/FAX: To have an Application for Diploma Replacement mailed or faxed to you, call 520/621-6056 Monday - Wednesday between 8:00 am - 1:30 pm.

• IN PERSON: Visit the Administration Building, room 313 between the hours of 8am - 5 pm to complete an Application for Diploma Replacement.

Fee: A replacement diploma is $35.00 (MasterCard, Visa, check or money order).

Size: 11" x 14"

Delivery time: 3 weeks

Name Changes
You must submit a copy of your marriage license along with an official name change form. Forms are available at the College of Medicine Student Records office (PO Box 245026, Tucson, AZ 85724-5026). New name diplomas are available at the $35.00 fee.
